Online Event Waiver

Before we get started with the fun, please read and agree to the terms below.

Release of Liability Waiver:

FOR MY GYM LIVE activities via Zoom: I understand that it is my responsibility to supervise my child(ren) during participation in My Gym LIVE classes, My Gym LIVE Virtual Birthday Parties, My Gym On-Demand video content, and any other content made available by MY GYM (collectively, “MY GYM Activities”).

I further understand and agree that I will be financially responsible for all costs incurred to treat any injuries that may occur during these activities, mobile, online or in-gym, regardless of whether my medical insurance would cover such charges and fees. I hereby give my consent to my child(ren)’s participation in MY GYM Activities and hold MY GYM harmless from any and all liability for any injuries or damages that my child(ren) or I may suffer in connection with MY GYM Activities.

Promotional, Website, and Social Media Notice: I hereby give MY GYM, and MY GYM ENTERPRISES, permission for the copyright and use of photos or videos they may capture of my child or me for use in various areas, including but not limited to the MY GYM website, training materials, promotional materials, marketing materials, and online. I hereby give MY GYM, and MY GYM ENTERPRISES, further permission for the copyright and use of these photos or videos on MY GYM’S social media websites.

Screen recording for My Gym LIVE birthday parties: I understand that MY GYM  may provide a screen recording of the MY GYM LIVE birthday party to be released to the family hosting the party and hold MY GYM and MY GYM ENTERPRISES harmless should my likeness or likeness of my child be used by another individual without my permission.
