Public health authorities, government officials, and our health and safety team continue to monitor health and safety conditions carefully and adapt to guidelines as necessary.
As we have done since we began our phased reopening, we’ve been very intentional and gradual in our approach to our COVID-19 health and safety protocols. Based on recent trends, guidance, and recommendations, and our concern for the health and safety of our My Gym families, we are requesting proof of vaccination preemptively for a possible future vaccination policy.

Below are our guidelines on the proposed policy and accepted forms of vaccination proof.
When Will This Policy Be Implemented?There is currently no estimated start date as we have not decided, nor are we yet required to, initiate a vaccination policy. |
What Counts as Fully Vaccinated?You are considered fully vaccinated against COVID-19 2 weeks after: You got a Johnson & Johnson (J&J)/Janssen COVID-19 vaccine, or; You got a second dose of a Pfizer or Moderna COVID-19 vaccine, or; You finished the series of a COVID-19 vaccine that has been listed for emergency use by the World Health Organization (WHO). |
Who Needs to Show Proof?At this time, no one is required to show proof of vaccination. However, the following is a proposed policy. "Anyone 12+ years old entering the building for ANY duration of time will need to show proof of their vaccination or submit proof in advance via the form below. This also extends to birthday party guests, seasonal party guests, camps, private playdates, and all other programs at My Gym Long Beach. At this time, children 11-years-old and younger do not need to show proof of vaccination."

What Forms of Vaccination Proof Are Accepted?We, here at My Gym Long Beach, accept the following types of vaccination proof: CDC COVID-19 Vaccination Record Card (white card); World Health Organization Vaccination Record Card (yellow card); Documentation of Vaccination from the Healthcare Provider entity that provided the COVID-19 vaccines; California Immunization Registry (CAIR2) Vaccination Record; Digital Vaccination Record or Digital Health Pass; International documents (for vaccinations administered outside the United States) Any of these types of proof may be shown as a physical document, a picture on a mobile device, or submitted in the form below.
in addition, PHOTO IDENTIFICATION A photo ID matching the name of the vaccination record is also required. The photo ID may be a: California or out-of-state driver's license or ID, Military ID, or Student ID Card (for children under 18-years-of-age). |
How Will I Show My Vaccination Proof?COVID-19 vaccine proof will only be checked once per person unless additional proof is needed or government/health officials require us to check different verification. The preferred method of getting your Vaccination/identification checked is through the submission form below. If you'd like, you can show us in person too. |
What Happens If I Do Not Have Vaccination Proof To Show/Submit?Again, at this time, no one is required to show proof of vaccination. However, the following is a proposed policy. "For the safety of our members and community, if anyone 12-years-old and older is not able to provide or declines to provide valid proof of COVID-19 vaccination, they will not be allowed to enter the gym. Exceptions may not be offered, and reasonable accommodations do not apply as MY GYM is considered a "private club" by the American Disability Act (ADA) and is therefore exempt from ADA Title III covering public accommodations and commercial facilities section III-1.6000." |
