Please note that:

- Parents are asked to stay at the gym floor with children for the play dates
- We will be offering a special during the play dates for those interested in enrolling
- Please RSVP below. If your name and info is in our system you won't need to complete all the fields 
- Play dates are age specific so we ask that participants are within the age range, that said sioblings are welcome
- There is no cost associated with this play date

Participating in any physical activity carries risks to the participant. It is required that the parent or guardian of any child(ren) participating in My Gym activities carefully reads and completes this release of liability in its entirety. 

Release of liability: In case of an emergency when I cannot be reached, I authorize the staff of My Gym to obtain whatever medical treatment deemed necessary for the welfare of my child(ren). I further understand and agree that I will be financially responsible for the charges and fees incurred in the rendering of such emergency treatment, regardless oof whether or not my medical insurance would cover such charges and fees. I hereby give my consent to my child(ren)'s participation in MY GYM activities and hold MY GYM harmless from any and all liability for any injuries or damages that I or my child(ren) may suffer in connection with the activities sponsored by MY GYM.
