I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.

You will automatically remain enrolled at My Gym until you cancel your enrollment and receive an email confirmation from the gym stating your cancellation request has been processed. The preferred method to request a cancellation is to do so from your My Gym account, but we will accept email, text, or in-person requests as well. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
